All participants in Gladiator athletics must have a valid physical form and insurance waiver on file with the Athletic Department. All physicals taken after April 1st are valid for the following school year. No physical form other than the Johns Creek physical form will be accepted. No student-athlete will be allowed to participate in open gym workouts without a valid physical and insurance form on file. All student-athletes must have a website permission form on file in order to appear in any photos on the various school websites.
